Enroll a Student

I hereby grant the Alaska Native Heritage Center (hereafter ‘Center’), and their respective agents and assignees, the right to use my name, likeness, image, voice, appearance and/or performance (hereafter ‘likeness’) created during my engagement with programming for advertising, publicity, trade or other lawful purposes involved in promoting the Center. I confirm that I have the right to enter into this Agreement, that I am not restricted by any commitments to their parties, and that the Center has no financial commitment or obligations to me as a result of this Agreement. I hereby give all clearances, copyright and otherwise, for use of my likeness. I expressly release and indemnify the Center and its officers, employees, agents and designees from any and all claims known and unknown arising out of or in any way connected with the above granted uses and representations. The rights granted the Center herein are perpetual and worldwide. Additionally, I hereby enter into this release of liability and waive any and all rights, claims, or causes of action of any kind arising out of my participation in the activities at the Alaska Native Heritage Center. Additionally, I waive any and all rights, claims, or causes of action that may arise out of my participation in activities operated by staff employed by the Alaska Native Heritage Center at off-site locations, including those which may be visited as part of travel programs. I am voluntarily participating in the activities at my own risk, and I understand that any injuries or outcomes that may arise from my own or others’ negligence are my own responsibility and that I assume all related risks. I have read the foregoing and understand its terms and stipulations and agree to all of them.

CONSENT: (If the person signing is under age 18, a parent or legal guardian must sign below.) I hereby certify that I am the parent or legal guardian of the model named above and I give my consent without reservation to the foregoing on behalf of him or her.
